Mike Lieberthal (1990) started at catcher.
Gavin Floyd (2001) came in as a relief pitcher, and later in the game both Pat Burrell (1998) and Chase Utley (2000) pinch hit in the pitchers spot.
While looking for this I also found this game where there were 3 Phillies first rounders, but also Eric Valent leading off for the Mets. That sort of thing is probably very common, especially with J.D. Drew out there.
Edit: Heres a better example from 2004.
Lieberthal started at Catcher, Burrell in LF (and went 4-4) and Floyd at SP. Utley pinch hit.
When I was going through this exercise I kept getting excited by Doug Glanville, who I had forgotten was drafted by the Cubs.
There are others but this might be the best one
All four stated this game (Along with Rollins and Howard)
I bet you could find with Brett Myers and Lieberthal between 2004 and 2006 that would meet this definition too.
